Lab slip

Also called: lab script, lab work order

A lab slip is the form a dental practice sends to a dental lab with a case. It carries the prescription, which names the patient, the teeth involved, the type of restoration, the material, the shade and the due date, with the dentist’s signature. Many practices now send the same details as a digital Rx.

How it shows up between practice and lab

On paper, the slip travels in the case pan with the impression or the model. The lab reads it when the case is checked in and works from it until the case ships.

A slip filled in between patients is easy to leave incomplete. A missing shade or due date sends the lab to the phone, and the case waits for the answer.

A paper slip also exists in one place at a time. The practice keeps a copy at best, so neither side sees a change the other makes.

A digital Rx carries the same fields with the scans attached, and both sides read the same version.
